  • Order an NGS test and send a tissue or blood sample to the lab

  • The lab will send the results to your office and bill the patient for the test
3. Patient Enrolls

After receiving EOB

  • Patient completes the NGS Affordability Enrollment form and submits required documentation to get reimbursed for out-of-pocket expenses

  • How much will commercially insured patients pay for their NGS biomarker test?

    With Biomarker Assist, eligible patients may pay as little as $0 out-of-pocket, up to the program maximum of $1000.*

  • Can I sign up for the Biomarker Assist NGS Affordability Program on the patient’s behalf?

    Unfortunately, you may not sign up for the program on the patient’s behalf. The patient is responsible for answering the co-pay eligibility questions and providing their attestation to confirm eligibility for the Biomarker Assist NGS Affordability Program. Please have your patient visit www.biomarkerassist.com or call us at (866) AMG - ASST (1-866-264-2778). We are available Monday through Friday, 9:00 am to 8:00 pm ET.

  • What are the payment terms for Biomarker Assist NGS Affordability Program?

    If the patient has paid their out-of-pocket responsibility to the lab, patient needs to complete enrollment, provide patient authorization, and send in an Explanation of Benefits (EOB) with proof of payment. If the patient has not paid their out-of-pocket responsibility to the lab, a patient needs to complete enrollment, provide patient authorization, and send in an EOB.

    The patient may enroll online or by downloading the form and sending it with required documents to Biomarker Assist via fax at 1-866-671-9373, or mail to ConnectiveRx, Attn Biomarker Assist NGS Affordability Program, PO Box 2355 Morristown, NJ 07962. Once received, Biomarker Assist will review the EOB form, determine your patient’s benefit, then send a check to the patient or lab with the appropriate amount. EOBs must be faxed to Biomarker Assist within 180 days of the date of service, otherwise your patient will not be eligible for this program and the patient will be responsible for all out-of-pocket costs.
